Adult’s Social Worker, Transitions team, Cheshire East 
Pay rate to £36 per hour 
Contract role – Transitions team 
 
Pertemps are recruiting for an experienced Adult’s Social Worker in the Crewe area. 
This is to work in a Transitions team 

• We assess all young people who are deemed to have need for care and support post 18 and who have an education health care plan (EHCP). 

• The worker should have knowledge of the care act, be able to competently complete assessments, support planning, mental capacity assessments, best interest decisions and possible court work. 

• There is plenty of hybrid working available.
