Hospital team Pathway team which involves Completing assessments, reviews, DST's, and panel Working in the office WED but will be required to travel to Slough to complete assessments. This is under strict timing to assess to enable people to be able to come off D2A and onto long term care if appropriate. Knowledge of Tech is needed Community work so access to transport needed.
