📍 Location: West Yorkshire
💷 Pay Rate: £35 per hour
🕒 Contract Type: Locum (Flexible Agency Role)

Are you an experienced and passionate Social Worker with a background in hospital discharge? We’re working with a local authority in West Yorkshire seeking skilled Qualified Social Workers to join their Hospital Discharge and Home First Team. This is a fantastic opportunity to support people transitioning from hospital to home, while working in a fast-paced, multidisciplinary environment.

🏥 About the Role
As part of the Hospital Discharge Team, you’ll help facilitate timely, safe discharges from acute settings — ensuring the right care and support is in place from day one.
You will:

Manage a caseload of adults with complex needs
Carry out strengths-based assessments and create tailored care and support plans
Deliver statutory responsibilities, including Care Act assessments, reviews, and safeguarding interventions
Work closely with health professionals, therapy teams, discharge coordinators, and care providers
Prioritise early intervention, recovery, and independence
Participate in a duty rota (1 week in every 4)

🧠 About You
To be successful in this role, you’ll need:
✔️ A recognised Social Work qualification (BSc, BA, MA, DipSW, CQSW)
✔️ Registration with Social Work England
✔️ Hospital discharge experience or equivalent acute-based adult services background
✔️ In-depth understanding of the Care Act, Mental Capacity Act, and safeguarding frameworks
✔️ Experience of managing a complex caseload and working under pressure
✔️ Strong communication, assessment, and decision-making skills
✔️ A commitment to person-centred, rights-based practice
